"How I Doctor Physician Award!" Nominate a doctor who deserves recognition

This survey is to nominate a doctor who deserves to be recognized for their work as a physician and/or their contributions to breaking out of the cookie cutter version of “what a doctor looks like” or “what a doctor’s supposed to do.” Medicine wasn’t built for creativity, but we think that’s exactly what it needs.
